To help your diagnosis and treatment in the ED, we ask you (or whoever accompanies you) to bring—if at all possible—the following information:

  • List of all medications, (please carry a copy of our Universal Medication Form with you at all times)

When you arrive, sign in at the reception desk.  The OMHS ED practices triage to evaluate the severity of your condition and the priority of treatment.  The outcome of your visit to the ED depends on the severity of your condition.  You may be treated and released, transferred to the right medical facility, or admitted.
